Finishing braces or clear aligner treatment is a milestone worth celebrating — but it is not quite the finish line. Retainers are the final and most important step in your orthodontic journey. They hold your teeth in their new positions while your jawbone, gums, and surrounding tissues fully stabilize — protecting the results you worked so hard to achieve. Why Do You Need a Retainer After Braces?

During orthodontic treatment, braces apply controlled force to move teeth through the surrounding bone into their corrected positions. Once braces are removed, the elastic fibers in the gums — which were stretched during treatment — naturally try to pull the teeth back toward their original positions. This is called orthodontic relapse, and it is one of the most common outcomes when retainers are not worn consistently. Continued jaw growth can also cause teeth to shift after treatment ends. A retainer holds the teeth firmly in place while these fibers relax and the bone around the teeth fully remodels and hardens. Without consistent retainer wear, teeth will begin to move — often within weeks of braces being removed. The risk of relapse is highest immediately after treatment and decreases over time, but teeth never become fully stable on their own. Retainer wear is a lifelong commitment.

Why Teeth Move After Braces Are Removed

Many patients are surprised to learn that teeth can shift even years after braces. There are two main reasons this happens:

  • Periodontal fiber memory: The elastic fibers in the gums and surrounding tissues have a “memory” of where the teeth used to sit. After braces are removed, these fibers exert a pulling force on the teeth. It takes months — sometimes over a year — for these fibers to fully relax and stop generating meaningful force.
  • Continued jaw growth and aging: The jaw continues to grow and change throughout life — even in adulthood. As the jaw naturally narrows with age, the front teeth can crowd and overlap over time, regardless of whether the patient had orthodontic treatment.

Both of these factors make ongoing retainer wear essential — not just for the first year after treatment, but for life. The good news: modern retainers are comfortable, discreet, and easy to incorporate into your nightly routine.

Types of Retainers at ABSC

Removable Retainers

Removable retainers are custom-made, thin, clear plastic devices that fit precisely over your teeth. They are similar in appearance to a sports mouthguard but far thinner and more discreet — most people will never notice you are wearing one. After your braces are removed, an impression of your teeth is taken and sent to a dental lab, where your retainers are fabricated to fit your exact smile.

Removable retainers are worn as instructed by Dr. Hessamfar. In most cases, patients wear them full-time for the first 10 days after braces are removed, then nightly for the long term. They should be removed when eating, swimming, or during vigorous physical activity.

Keep your retainer in its case whenever it is not in your mouth. Retainers left on tables, wrapped in napkins, or placed loose in bags are frequently lost or accidentally thrown away — and replacement retainers take time and incur an additional cost.

Fixed (Bonded) Retainers

A fixed retainer — also called a bonded retainer — consists of a thin stainless steel wire permanently bonded to the back of the front teeth. It is completely invisible, unobtrusive, and requires no action from the patient to be effective. Most patients forget it is there within days of placement.

At ABSC, fixed retainers are typically fitted immediately after braces are removed — before impressions are taken for the removable retainer. Fixed retainers provide continuous, passive retention 24 hours a day without any compliance requirement.

Fixed retainers do require extra care during brushing and flossing — a floss threader or water flosser is needed to clean beneath the wire. Dr. Hessamfar and the ABSC team will demonstrate proper cleaning techniques at your fitting appointment.

Many patients wear both a fixed retainer and a removable retainer — the fixed retainer provides continuous protection while the removable retainer is used nightly for added security, particularly for the back teeth.

When and How Long to Wear Your Retainer

Follow Dr. Hessamfar’s specific instructions for your retainer schedule. The standard guidance for most patients is:

  • First 10 days after braces removal: Wear your removable retainer full-time (day and night), removing only to eat, brush, and floss
  • After the first 10 days: Wear your retainer every night while sleeping — indefinitely
  • Always remove your retainer when eating — food and chewing forces can crack or warp the plastic
  • Remove your retainer for swimming and contact sports — water and physical impact can damage or dislodge it
  • Never skip nights — even occasional gaps in wear allow small amounts of movement that can accumulate over time. If your retainer feels tight after not wearing it for a few days, that is a sign your teeth have already begun shifting

Retainer wear time is a lifelong commitment for most patients. Some patients may require permanent retention — this will be discussed as part of your treatment plan with Dr. Hessamfar. Remember: your retainer only works when you are wearing it.

How to Care for Your Retainer

Proper retainer care keeps it clean, odor-free, and functioning correctly for as long as possible.

  • Clean your retainer daily — use a soft toothbrush with a small amount of mild liquid soap (dish soap or hand soap work well). Avoid toothpaste, which is abrasive and can scratch clear plastic
  • Rinse thoroughly with cool water after cleaning — never use hot water, which can warp clear plastic retainers
  • Soak occasionally in a retainer cleaning tablet solution (such as Retainer Brite or Efferdent®) to remove buildup and eliminate bacteria — soaking supplements daily brushing, it does not replace it
  • Never flip your retainer with your tongue while wearing it — this habit puts stress on the appliance and can crack it, and can also cause unwanted tooth movement
  • Never leave your retainer in direct sunlight, in a hot car, or near a heat source — and never boil it to sterilize it. Heat and boiling water permanently distort the plastic. If you feel your retainer needs professional disinfection, bring it to any ABSC office and we will clean it for you at no charge
  • Keep it away from pets — dogs in particular are attracted to the smell of retainers and will chew them
  • Always store in the protective case provided by ABSC when not being worn
  • Bring your retainer to every follow-up appointment so Dr. Hessamfar can check its fit and condition

Soreness and Adjustment Period

It is normal to experience mild soreness and slight speech changes — such as a temporary lisp — when you first begin wearing your retainer. These issues resolve on their own within a few days as your mouth adjusts to the appliance. Speaking aloud, reading out loud, or practicing specific sounds can speed up the adaptation process.

If your retainer feels noticeably tight or uncomfortable after a period of not wearing it, wear it gently for a few nights and the tightness should ease. However, if it feels extremely tight, painful, or no longer fits properly, contact ABSC before forcing it back in — Dr. Hessamfar will assess whether an adjustment or a replacement is needed.

Never try to adjust or bend your retainer yourself. If you experience persistent discomfort, unusual pain, or notice damage to the retainer wire or plastic, contact our office promptly.

Lost or Broken Retainer? Act Quickly.

If your retainer is lost, broken, or no longer fits, contact ABSC as soon as possible. Every day without a retainer is a day your teeth can shift. A new retainer can be fabricated quickly, but there will be a replacement fee. The sooner you contact us, the less movement will have occurred by the time your new retainer is ready.

To minimize the risk of losing or breaking your retainer:

  • Always use the protective case — never wrap in tissues or leave loose in bags
  • Keep it away from children and pets
  • Never bend or force the retainer if it feels tight — call us instead
